Designed for individuals who want a focused medical visit for single concerns, second opinions, or situations where clarity and guidance are needed without long-term commitment. Some patients use à la carte care once, while others begin here and later transition into a Membership or Protocol.

Clinician Preparation
Before your visit, relevant medical history is gathered, including prior labs, imaging reports, and any outside evaluations. These materials are reviewed in advance, allowing Jeremy to come prepared.

Time is spent understanding the full picture, talking through options, and making decisions together, ending with clear, actionable next steps.

Follow-up Support
If needed, additional support may include testing, labs, imaging, prescriptions, supplements, or HBOT sessions. Recommendations will be tailored to your situation.

Clinical Visit
$400 | 40 minutes
Your visit may be conducted in person or via telemedicine, depending on the need.
À la carte care stands on its own and does not require enrollment in a Membership.
For patients who need structured, time-bound care, Protocols may be recommended. For those who value continuity, Concierge Medicine Memberships offer a different care model built around relationship, access, and longitudinal care.

Note: These services are billed separately for à la carte patients. When included as part of a Protocol, they are bundled into Protocol pricing. When clinically appropriate, they are included for Members.
Hyperbaric oxygen therapy (HBOT) may be recommended following clinical evaluation as part of a personalized medical plan.
For individuals seeking focused support, HBOT can be utilized outside of a formal Protocol or Membership; however, it is still provided within a provider-directed framework that includes appropriate screening, oversight, and alignment with a defined clinical objective.
When indicated, care may involve a series of sessions over a defined period, with frequency and duration guided by your goals and clinical response.
HBOT may also be incorporated into Protocol-based or Membership care when coordination with other interventions is beneficial.
HBOT is provided within a medical framework and is not offered as a self-directed service.
